Concierge Practice, Individualized Care, Evidence-Based Medicine.

My personal and professional aim is to form a patient-doctor relationship that improves your health and quality of life. That relationship is more than a service or commodity and the value of it is measured by your health and well-being over time.

The structure of my practice is designed to afford us both the time necessary to get to know you, to understand your medical history, and to understand what you value most in a successful relationship with your doctor and the healthcare system. While health insurance of some type remains a necessity for most people, it is almost unfeasible for good physicians to devote the vital time to thoroughly address a patient’s needs while relying on insurance reimbursement to cover the operating costs of a primary care medical practice. In our model, the nuance of excellent care is not lost in the bureaucracy of medicine. And we aren’t required to waste time and resources proving it to for-profit insurers. You and I will work together to achieve your goals.

When I become your doctor, we will set a first two-hour appointment to learn your medical history and conduct a physical examination. I will compile your medical records and test results from your other doctors and/or institutions and create a plan to address your ongoing medical conditions as well as create a preventative care plan. We will schedule follow-up appointments regularly that fit your schedule -- and are able to conduct them virtually on-line if needed. I will also coordinate your care with existing sub-specialists and make new referrals as needed. You can expect a full hour at least once a year for an annual physical exam.

My practice keeps the number of visits each day low so that the work behind the scenes to coordinate your care can be done thoughtfully and thoroughly. It also allows time for follow-up phone calls and to address new or urgent issues. At night, on weekends, or holidays, you can call me.

  • Health Insurance

When I become your doctor, you will reimburse me directly. I am not paid nor incentivized by any other entity to provide your medical care. None of the services that I provide in my office as your primary care doctor are billed to your insurance, and they are all included in your annual membership fee that you pay directly to me.

I recommend you maintain healthcare insurance sufficient to cover emergency room visits, hospitalizations, and depending on your personal situation, laboratory and other diagnostic imaging tests. Health insurance plans’ coverage of medications varies widely.

  • Why a Hospitalist is important

While I keep sufficient staff privileges to access my patients’ records at the two local hospital systems, I no longer attend patients’ cases directly. Should the time come that you require hospitalization, your care team will be led by a Hospitalist, a physician dedicated to managing your inpatient care. I will be supporting that team and will remain available to communicate with you and your other physicians.

  • Advanced care planning

A component of your ongoing care also includes planning for the future. Formalizing your intentions, should you be incapacitated to communicate them yourself near the end of life or during an unexpected serious illness, is an important part of completing your medical record. Equally important is facilitating a dialogue about your wishes with those you love. I’m happy to help you complete these tasks.
